    Youth Unemployment Challenges in Mining Areas of Ghana

    Ghana, like the rest of West Africa is experiencing tremendous human migration both internally and across international boundaries. Rural-urban migration has assumed uncontrollable dimensions in the sub-region and the social consequences have become major development challenge. In Ghana the mining communities have been at the receiving end for some time now. This study on the Obuasi Municipal Assembly (OMA) in the Ashanti region of Ghana explores the tremendous socioeconomic changes, especially demographic patterns as a result of the inflows of migrants into the Obuasi Township and its catchment area in search of non existing jobs especially in mining. A major outcome is the serious unemployment problem in the township with all the attendant social vices. A three-month socio-economic study of the municipality was carried out to determine the scope of unemployment. The study showed that there is acute unemployment situation in the municipality which is due to the fact that AngloGold Ashanti, a mining giant in Ghana, the major employer, has limited job openings especially for menial workers who flock to the company. Other income generating opportunities are few. Agriculture which has the capacity to employ majority of the unemployed youth does not appeal to them because it is considered not lucrative. The acute unemployment situation has contributed significantly to the high crime rate, prostitution and widespread illegal mining activities with their attendant problems. The study explores options that are feasible for a typical mining setting especially for the youth who are very vulnerable and susceptible to crime and other social vices. Job creation, through the development and implementation of sustainable programmes aimed at training the youth to acquire the necessary employable skills is one of the options considered by the municipal managers and their partners.     Aesthetic outcome and the need for revision of unilateral cleft lip repair at Komfo Anokye Teaching Hospital

    Background: The Millard method of unilateral cleft lip repair has been  associated with a short lip and a flattened nose on the cleft side. The aim of this study was to determine the need for revision surgery followingrepair of unilateral cleft lip repair at the Komfo Anokye Teaching HospitalMethod: Satisfaction with facial appearance (upper lip, nose and general facial appearance) was assessed quantitatively by means of a Visual Analogue Scale (VAS), where 0cm indicates totally unsatisfied or “highly unattractive” and 10cm indicates totally satisfied or “highly attractive”. Three assessors - parents, surgeon and layperson - were purposively selected to score their level of satisfaction with repair of complete and incomplete unilateral cleft lip. The assessors also indicated the need for any revision.Results: The total sample size was 120, of which 40.0% were male and 60.0% were female. There were 79 cases of repaired complete unilateral cleft lip and 41 incomplete unilateral cleft lip. Average scores of satisfactionof parents were 6.6, 6.8 and 7.2 for nose, lip and general facial  appearance (GFA) respectively. Satisfaction scores for surgeon were  6.1(nose), 6.0(lip) and 6.5(GFA), while those of the lay-assessor were5.2(nose), 5.4 (lip) and 6.0(GFA). Concerning the need for revision, parents indicated 30.2% as needing revision, surgeon 33%; and  lay-assessor 40%. Of the cases that needed revision, 33.3% were complete cleft lip and 0.1% were incomplete cleft lip.Conclusion: Parents were more satisfied with unilateral cleft lip repair using the Millard procedure than either the surgeon or lay assessor. Those who needed revision were mostly children who presented with complete unilateral cleft lip

    Impurity and quaternions in nonrelativistic scattering from a quantum memory

    Models of quantum computing rely on transformations of the states of a quantum memory. We study mathematical aspects of a model proposed by Wu in which the memory state is changed via the scattering of incoming particles. This operation causes the memory content to deviate from a pure state, i.e. induces impurity. For nonrelativistic particles scattered from a two-state memory and sufficiently general interaction potentials in 1+1 dimensions, we express impurity in terms of quaternionic commutators. In this context, pure memory states correspond to null hyperbolic quaternions. In the case with point interactions, the scattering process amounts to appropriate rotations of quaternions in the frequency domain.     The use of free autogenous rib grafts in maxillofacial reconstruction

    Background: Hard tissue defects in the maxillofacial region due to trauma or ablative surgery result in functional and cosmetic problems. State-of-theart methods for reconstruction include the use of vascularised tissue. Objective: To review our results with the use of non-vascularised rib grafts for maxillofacial reconstruction. Method: Patients who underwent maxillofacial reconstruction using rib at the Komfo Anokye Teaching Hospital during 1996-2004 were studied. The technique for rib harvest and implantation of the graft was standardized. Clindamycin was administered peri-operatively and the harvested rib was temporarily stored in clindamycin/saline before implantation. The graft was successful if it survived beyond 6 months after placement. Follow- up was for at least 12months postoperatively. Results: A total of 29 patients were studied. The indications for grafting included ameloblastoma, malignant disease, cyst, ankylosis, and trauma. Either rib bone only or with cartilage were used. In 90% of patients (26/29) the graft healed uneventfully. Two patients had dehiscence of the wound with exposure of the graft intraorally within two weeks of surgery and were successfully managed with antibiotics. Conclusion: Free autogenous rib was successfully used to reconstruct defects in the maxillofacial region. Further stabilization of the graft by intermaxillary fixation and the prophylactic use of clindamycin may have helped to minimize complications

    Individual and medical characteristics of adults presenting to an urban emergency department in Ghana

    Background: The aims of this study were to characterize the patients seeking acute care for injury and noninjury complaints in an urban Emergency Department in Ghana in order to 1) inform the curriculum of the newly developed Emergency Medicine resident training program 2) improve treatment processes, and 3) direct future community-wide injury prevention policiesStudy Design: A prospective cross-sectional survey of patients 18 years or older seeking care in an urban Accident and Emergency Center (AEC) was conducted between 7/13/2009 and 7/30/2009. Questionnaires were administered by trained research staff and each survey took 10-15 minutes to complete. Patients were asked questions regarding demographics, overall health and chief complaint.Results: 254 patients were included in the sample. Participants’ chief complaints were classified as either medical or injury-related. Approximately one third (38%) of patients presented with injuries and 62% presented for medical complaints. The most common injury at presentation was due to a road traffic injury, followed by falls and assault/fight. The most common medical presentation was abdominal pain followed by difficulty breathing and fainting/ blackout. Only 13% arrived to AEC by ambulance and 51% were unable to ambulate at the time of presentation.Conclusion: Approximately one-third of non-fatal adult visits were for acute injury. Future research should focus on developing surveillance systems for both medical and trauma patients. Physicians that are specifically trained to manage both the acutely injured patient and the medical patient will serve this population well given the variety of patients that seek care at the AEC.Keywords: Emergency Medicine, Injury, Surveillance, Ghana, Characteristic

    Assessment of emergency medical services in the Ashanti region of Ghana

    Background: We aimed to assess the structure, function and performance of Ashanti Region’s emergency medical services system in the context of the regional need for prehospital emergency care.Design: A mixed-methods approach was employed, using retrospective collection of quantitative data and prospectively gathered qualitative data. Setting – pertinent data were collected from Ghanaian and international sources; interviews and technical assessments were performed primarily in the Ashanti Region of Ghana.Participants: All stakeholders relevant to emergency medical services in the Ashanti Region of Ghana were assessed; there was a special focus on National Ambulance Service (NAS) and Ashanti Region healthcare personnel.Intervention: This was an observational study using qualitative and quantitative assessment techniques.Main outcome measures: The structure, function and performance of the Ashanti emergency medical services system, guided by a relevant technical assessment framework.Results: NAS is the premier and only true prehospital agency in the Ashanti Region. NAS has developed almost every essential aspect of an EMS system necessary to achieve its mission within a low-resource setting. NAS continues to increase its number of response units to address the overwhelming Ashanti region demand, especially primary calls. Deficient areas in need of development are governance, reliable revenue, public access, community integration, clinical care guidelines, research and quality assurance processes.Conclusions: The Ashanti Region has a growing and thriving emergency medical services system.     Hiv/Aids prevalence at the accident & emergency centre of a tertiary and referral health institution in Ghana

    Background: Ghana has an estimated HIV prevalence of 1.4%, but the HIV prevalence of patients presenting at emergency departments in Ghana is not well documented in published literature. This study evaluated the prevalence of HIV infection at the Accident & Emergency Department, Komfo Anokye Teaching Hospital (KATH A&E), Kumasi, Ghana.Methods: A descriptive cross-sectional survey was carried out on patients aged 18 and above presenting to KATH A&E. An opt-in testing approach was used; consenting patients were screened for HIV using rapid HIV finger-stick testing with HIV 1-2 STAT-PAK. Sero-positivity was confirmed by OraQuick HIV 1-2 test. Data was analysed using multivariate logistic regression.Results: 1125 patients presenting at the KATH A&E during the study period were offered the Rapid HIV test. 667 of these patients consented to have the test. HIV prevalence was 13.5% (90/667). 53 females (58.9%) were HIV positive compared to 37 males (41.1%). The age group 30-50 years had the highest risk of being HIV-positive. Other socio-demographic variables such as educational level and occupation were significantly associated with HIV-infection (Pvalue = 0.001 at 95% CI).Conclusion: This study shows that emergency department HIV testing in Ghana is feasible. The prevalence of HIV sero-positive patients presenting at KATH A&E was tenfold higher than national estimates. We conclude that this study showed a high prevalence among patients seeking emergency care in our setting. Testing in the emergency department could lead to early detection of HIV-infected patients for linkage to care.Keywords: HIV Infections; HIV Screening; Prevalence, Diagnosis, Emergency Departmen

    Teleportation, Braid Group and Temperley--Lieb Algebra

    We explore algebraic and topological structures underlying the quantum teleportation phenomena by applying the braid group and Temperley--Lieb algebra. We realize the braid teleportation configuration, teleportation swapping and virtual braid representation in the standard description of the teleportation. We devise diagrammatic rules for quantum circuits involving maximally entangled states and apply them to three sorts of descriptions of the teleportation: the transfer operator, quantum measurements and characteristic equations, and further propose the Temperley--Lieb algebra under local unitary transformations to be a mathematical structure underlying the teleportation. We compare our diagrammatical approach with two known recipes to the quantum information flow: the teleportation topology and strongly compact closed category, in order to explain our diagrammatic rules to be a natural diagrammatic language for the teleportation.Comment: 33 pages, 19 figures, latex.
